05/08/2022: PGS. Đào Việt Hằng (ĐH Y Hà Nội), Updated Hepatocellular Carcinoma Managementand Potential Research Directions onHepatocellular Carcinoma Prevention

HCC is a major burden disease in Vietnam, ranking the first in prevalence and mortality of malignancies. Using ultrasound plus biomarker Alpha- fetoprotein (AFP) plays an important role in HCC screening for high-risk populations such as cirrhosis, viral hepatitis,… HCC diagnosis still faces many difficulties due to the heterogeneous quality of diagnostic imaging facilities and many new but difficult-to-access biomarkers. The combination of triple biomarkers AFP, AFP-L3, and PIVAKII is expected to improve the early diagnosis of HCC. Different imaging classifications such as mRECIST, LIRAD, and RECIL are recommended to achieve a standardized and homogenous evaluation of liver tumors. The Barcelona staging helps to select the optimal treatment approach and predict survival time for the patient. HCC prevention strategy requires a multimodal approach: prevention of risk factors, especially hepatitis virus, screening in high-risk populations, and development of techniques for early diagnosis of both new and recurrent cases combining both biomarkers and new technologies in diagnostic imaging.
